Accident Incident or Unsafe Condition

Accidents, incidents, or unsafe conditions on campus should be reported to EH&S as soon as practible. Reports may be made in one of two ways:

• By filling out and submitting an Accident/Incident/Unsafe Condition Form   on line, or
  
  
• Printing and filling out a copy of the  .pdf format Accident/Incident/Unsafe Condition Form and faxing it to the EH&S Office at (479)575-6474.
